Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate prior and current medical history, ongoing medical conditions, and medication history during each visit
• Offer patient education on chronic conditions, medications, and preventive care measures
• Identify potential care gaps and recommend actionable solutions to enhance patient outcomes
• No prescribing of medications or creation of treatment plans required

Qualifications:

• Active Nurse Practitioner License in New Jersey
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• MSN from an accredited nursing program
• Minimum six months of current Nurse Practitioner experience preferred
