Deliver high-quality care and empower your schedule as you serve the community through comprehensive home health risk assessments. This per diem nurse practitioner opportunity involves conducting in-home annual health assessments and post-discharge visits for Medicare Advantage plan members across Hudson County, NJincluding Jersey City, Bayonne, Union City, and nearby communities.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform thorough annual home health risk assessments for members of a Medicare Advantage plan
• Review and evaluate both previous and current medical history, existing conditions, and medication lists
• Educate patients on their health needs, conditions, and medications to encourage optimal wellness
• Detect gaps in care; provide actionable recommendations to support care continuity and improved outcomes
• Focus exclusively on assessmentno procedures, treatment plans, or prescribing required

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board Certification from AANP or ANCC
• Current CPR (BLS) certification
• At least 6 months of recent NP experience (preferred)
• MSN from an accredited nursing program
