Job Description

Deliver high-quality care to Medicare Advantage plan members as a Nurse Practitioner providing home health risk assessments across Union County, NJ. This per diem opportunity allows professionals to create their own schedule with flexible shifts available between Monday and Sunday. Responsibilities:

  • Conduct in-home health risk assessments for Medicare Advantage members
  • Evaluate prior and current medical history, ongoing health conditions, and medication history
  • Educate patients and caregivers about their medical conditions and proper medication use
  • Identify care gaps and provide recommendations for improvement
  • Focus on evaluationsno requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in New Jersey
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • Minimum of six months experience as an NP preferred
  • MSN degree from an accredited nursing school

This role offers compensation of $80-$90 per evaluation, in addition to a mileage stipend and payment for administrative time.
