Step into a vital position where youll deliver comprehensive care to individuals in their homes, supporting health and wellbeing throughout the Newark and Morris County area. This per diem opportunity is ideal for experienced Nurse Practitioners seeking a flexible, rewarding, and independent environment.
Schedule:
- Flexible shifts available Monday through Sunday
- Create a personalized schedule to fit your lifestyle
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Complete home health risk assessments for Medicare Advantage members in Morris County, including areas such as Parsippany-Troy Hills, Mount Olive, and Randolph
- Review past and present medical history, chronic conditions, and medication use
- Offer patient-centered education regarding healthcare conditions and treatment compliance
- Identify gaps in care and provide clear recommendations for improvement
- No need to create extended treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- Minimum of six months experience as a Nurse Practitioner preferred
- MSN from an accredited nursing program
