Advance your career as a Nurse Practitioner by conducting home health risk assessments for Medicare Advantage plan members across Hudson County, NJ. This per diem opportunity allows you to make a significant impact by evaluating patients in the comfort of their homes, with flexibility to create your own schedule. Locations can include Jersey City, Bayonne, and Union City, ensuring a dynamic and rewarding experience with each shift.
Schedule:
- Flexible shifts available between Monday and Sunday
- Set your own hours and build a personalized work schedule
- Choose a minimum weekly or bi-weekly hourly commitment that suits your lifestyle
Key Responsibilities:
- Perform annual health risk assessments for Medicare Advantage members in home settings
- Review and document prior and current medical history, conditions, and medication regimens
- Educate patients about their health conditions and prescribed medications
- Proactively identify gaps in care and offer recommendations for improvement
- No requirements for creating treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current BLS certification (CPR)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent NP experience is preferred
- MSN from an accredited nursing program
Compensation for this role includes $80$90 per in-home evaluation, plus a mileage stipend and administrative time.
