Job Description

Experienced Nurse Practitioners are sought for per diem, home-based health assessments with a focus on flexibility and patient-centered care. This opportunity centers on conducting home health risk evaluations for Medicare Advantage members in Sharp County, including areas such as Cherokee Village, Horseshoe Bend, and Cave City. Each assessment visit averages about 45 minutes, varying from annual evaluations to post-discharge assessments.

Schedule:

  • Flexible shifts offered any day of the week, Monday through Sunday
  • Opportunity to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review patient medical history and current conditions, including medication management
  • Educate patients on their health conditions and prescribed medications
  • Identify gaps in care and recommend appropriate follow-up actions
  • Note: This role does not include treatment planning or prescription writing

Desired Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS required)
  • MSN degree from an accredited nursing school
  • At least 6 months of recent Nurse Practitioner experience preferred

Practitioners are compensated at $80$90 per evaluation, with additional mileage stipend and coverage for administrative time.
