This per diem opportunity offers highly flexible scheduling, allowing professionals to choose shifts that best suit their availability, with shifts available any day of the week.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct home health risk assessments for Medicare Advantage plan members during in-person visits
- Evaluate prior and current medical history, medical conditions, and medication use
- Educate patients on their conditions and medications during each assessment
- Identify care gaps and provide actionable recommendations for improved health outcomes
- No treatment plan development or medication prescribing required
Qualifications:
- Active Nurse Practitioner license in New York
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- At least 6 months of recent NP experience is preferred
- MSN degree from an accredited institution
Compensation for this position is $80$90 per evaluation, in addition to mileage reimbursement and payment for administrative time.
