Each assessment averages about 45 minutes and could range from annual evaluations to post-discharge visits.
Schedule:
- Flexible shifts available Monday through Sunday
- Ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members
- Review and evaluate both prior and current medical histories, existing medical conditions, and medication usage
- Educate patients on medical conditions and medication management
- Detect gaps in care and offer practical recommendations for improved health outcomes
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in New York
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months of recent experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
