Job Description
Deliver essential care and make a meaningful impact as a Nurse Practitioner conducting in-home health risk assessments for Medicare Advantage plan members. This per diem opportunity serves the Bronx, NY community and features flexibility, autonomy, and the chance to tailor your schedule to meet your needs.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to best fit your lifestyle
- Minimum commitment of hours weekly or bi-weekly
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical conditions, medication history, and overall health status during each visit (approx. 45 minutes per patient)
- Educate members and their caregivers on their health conditions and prescribed regimens
- Identify care gaps, and offer guidance or recommendations to address those needs
- Visits may include annual assessments or post-discharge appointments; no treatment planning or prescription duties required
Qualifications:
- Active Nurse Practitioner license in New York
- Current CPR certification (BLS required)
- Board certification from AANP or ANCC
- A minimum of 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
