Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own schedule with minimum weekly or bi-weekly commitments

Key Responsibilities:

• Perform thorough annual home health risk assessments for Medicare Advantage health plan members
• Review and evaluate patients prior and current medical history, medical conditions, and medication use
• Deliver clear education on medical conditions and prescribed medications
• Identify gaps in care and offer actionable recommendations for care coordination and improvement
• Focus is exclusively on assessments: no treatment planning or medication prescribing is involved

Qualifications:

• Active Nurse Practitioner license in New York
• Current CPR certification (BLS required)
• Board certification via AANP or ANCC
• Minimum of 6 months' experience as a Nurse Practitioner preferred
• MSN from an accredited nursing program

This per diem position provides competitive compensation, offering $80-$90 per completed evaluation. Additionally, youll receive a mileage stipend and compensation for administrative timeensuring your efforts are valued both in and out of the field.
