Enjoy flexibility and a rewarding scope of practice as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, youll conduct in-home evaluations for Medicare Advantage plan members within Westchester County, including Yonkers and surrounding areas such as New Rochelle, Mount Vernon, and White Plains. Each visit, averaging approximately 45 minutes, will offer unique varietyranging from annual health assessments to post-discharge follow-ups.

Schedule:

• Flexible shifts available seven days a week
• Design your own work schedule
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Complete comprehensive annual and post-discharge home health risk assessments
• Review and document medical history, current conditions, and medication use
• Educate members on their health conditions and prescribed medications
• Identify and report on gaps in care with actionable recommendations
• Note: No prescribing or development of treatment plans is required

Qualifications:

• Active Nurse Practitioner license in New York
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited nursing program
• At least 6 months of NP experience preferred

Youll receive competitive compensation of $80$90 for each evaluation, including a mileage stipend and payment for administrative time.
