A rewarding per diem opportunity is available for an experienced Nurse Practitioner to provide home health risk assessments throughout Westchester County, NY, including locations such as Yonkers, New Rochelle, Mount Vernon, and White Plains. This role focuses on delivering quality, patient-centered assessments for Medicare Advantage plan members during annual evaluations or post-discharge visits.
Schedule:
- Flexible scheduling options are available between Monday and Sunday
- Create and manage your own schedule to best fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Evaluate patients medical history, current conditions, and medication usage
- Educate individuals on medical conditions and medication regimens
- Identify gaps in care and provide evidence-based recommendations for resolution
- No need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in the state of New York
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- At least 6 months of current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
