Make a meaningful impact by delivering patient-centered care in the comfort of individuals homes as a Nurse Practitioner specializing in home health risk assessments throughout Dutchess County, NY. In this per diem role, you'll perform comprehensive evaluations for Medicare Advantage plan members, guiding patients toward improved health outcomes and closing gaps in care.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to suit your lifestyle
- Meet minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct annual or post-discharge home health risk assessments, with each visit averaging 45 minutes
- Evaluate patients medical history, current conditions, and medication profiles
- Provide clear education to patients regarding diagnoses and medication regimens
- Identify gaps in care and make actionable recommendations to support patient health
- Focus on assessmentsno treatment planning or prescription writing required
Desired Qualifications:
- Active Nurse Practitioner license in New York State
- Certification in Basic Life Support (BLS/CPR)
- Board certification through AANP or ANCC
- At least six months of experience as a Nurse Practitioner is preferred
- Graduate of an accredited MSN program
