Job Description
Deliver vital healthcare services to Medicare Advantage members by performing in-person home health risk assessments across Washington County, Texas. As a Nurse Practitioner in this per diem position, youll play a key role in supporting preventive care while enjoying an exceptionally flexible schedule tailored to your preferences.
Schedule:
- Flexible shifts available any day of the week, Monday through Sunday
- Set your own work hours
- Choose your minimum weekly or bi-weekly hourly commitment
Core Responsibilities:
- Conduct comprehensive home health risk assessments, typically 45 minutes each, at member residences in areas such as Brenham, Mount Pleasant, Winfield, and Talco
- Review and document prior/current medical history, chronic conditions, and medication profiles
- Educate patients regarding health conditions and proper medication usage during home visits
- Identify gaps in care, offering actionable recommendations to improve health outcomes
- Perform visits focusing solely on assessment and education: no ongoing treatment plans or medication prescribing required
Qualifications:
- Valid Nurse Practitioner license in Texas
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- At least six months of recent nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
