Deliver high-quality, patient-centered care in a flexible per diem role as a Nurse Practitioner specializing in in-home health risk assessments. In this position, you will make a meaningful impact for Medicare Advantage plan members throughout Claiborne County, meeting patients in their homes across a variety of communities. Enjoy the autonomy to create your own schedule, choosing shifts between Monday and Sunday to balance your professional and personal commitments.
Schedule:
- Flexible shifts available seven days a week
- Design your own schedule for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment
Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and evaluate each patients medical history, current conditions, and medication profiles
- Educate patients regarding their medical conditions and medications to enhance understanding and compliance
- Identify care gaps and offer recommendations for improved patient outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification as a Nurse Practitioner through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least 6 months of current NP experience preferred
