Deliver essential patient-centered care in a dynamic home health setting as a Nurse Practitioner conducting in-home risk assessments. This per diem opportunity offers exceptional flexibility in scheduling, allowing you to select shifts that align with your availability across the weekMonday through Sunday. You can determine your own schedule with a minimum weekly or bi-weekly hourly commitment, providing an ideal balance for work and personal life.
Responsibilities include:
- Completing annual home health risk assessments for Medicare Advantage plan members
- Evaluating patients prior and current medical history, ongoing medical conditions, and medication usage
- Offering education regarding medication regimens and various health conditions to patients and families
- Identifying potential gaps in care and providing recommendations to address these gaps
- Performing assessments within homes located throughout Payette County, including Payette, Fruitland, and New Plymouth
- Focusing exclusively on assessmentsno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board Certification (AANP or ANCC)
- Current CPR (BLS) certification
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
Compensation is provided at $80$90 per evaluation, with additional mileage stipend and compensation for administrative time.
