Job Description

Deliver vital care in the community as a Nurse Practitioner specializing in home health risk assessments. This per diem opportunity enables you to make a direct impact on Medicare Advantage plan members in Payette County, including the areas of Payette, Fruitland, and New Plymouth. You will have the autonomy to create your own schedule, selecting flexible shifts between Monday and Sunday, while helping to improve patient outcomes right in their homes.

Schedule:

  • Flexible shifts offered seven days a week (Monday through Sunday)
  • Ability to customize your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Perform annual home health risk assessments on Medicare Advantage plan members
  • Review and evaluate medical history, current conditions, and medication usage
  • Educate patients about medical conditions, treatment adherence, and medication information
  • Identify care gaps and recommend steps for resolution
  • Focus exclusively on assessmentno treatment planning or medication prescribing involved

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Board Certification via AANP or ANCC
  • Current CPR/BLS certification
  • MSN degree from an accredited nursing program
  • At least six months of recent NP experience strongly preferred

You will be compensated $80$90 for each evaluation conducted, plus a mileage stipend and administrative time, ensuring your expertise is valued and your travel is supported.
