Job Description
Seeking a dedicated Nurse Practitioner to perform home health risk assessments for Medicare Advantage members throughout Allen County, IN, including Fort Wayne, New Haven, and Huntertown. This per diem opportunity provides exceptional flexibilitydesign your own schedule with shifts available seven days a week and minimal weekly or bi-weekly commitments.
Schedule:
- Flexible shifts between Monday and Sunday
- Choose your own hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive home health risk assessments, each averaging 45 minutes, for Medicare Advantage members in their homes
- Assess prior and current medical history, chronic conditions, and medication usage
- Educate patients about their diagnoses, treatment options, and medication management
- Identify care gaps and recommend follow-up actions or resources
- Complete documentation without the need to develop treatment plans or prescribe medications
Qualifications:
- Licensed Nurse Practitioner (NP) in the state of Indiana
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- Minimum of six months' current NP experience preferred
