Job Description

A rewarding per diem opportunity is available for an experienced Nurse Practitioner with a background in home health to support Medicare Advantage plan members. In this role, you will conduct comprehensive home health risk assessments across Adams County, including Decatur, Berne, and Geneva. These visits, typically lasting 45 minutes, may range from annual health assessments to post-discharge check-ins, offering a blend of clinical evaluation and patient education.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Choose minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Complete annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, medical conditions, and medication use
  • Educate patients about their health conditions and prescribed medications
  • Identify and address any gaps in care, providing recommendations for follow-up when needed
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Board certification (AANP or ANCC)
  • Completion of MSN from an accredited nursing school
  • Current CPR certification (BLS)
  • At least 6 months of recent NP experience preferred

Compensation:

  • Earn $80-$90 per completed evaluation, with an additional mileage stipend and paid administrative time
