Bring your clinical expertise to a flexible, patient-centered position as a Nurse Practitioner, delivering home health risk assessments across New Haven County. This per diem opportunity empowers advanced practice clinicians to create their own schedules and support Medicare Advantage plan members through in-home assessments that focus on preventive care and care coordination.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule to suit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments in patients homes
- Review prior and current medical histories, medical conditions, and comprehensive medication regimens
- Educate patients on disease processes and safe medication use
- Identify gaps in care and provide actionable recommendations for improvement
- Assessments may include annual wellness visits and post-discharge evaluations
- Note: This position does not require developing treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Connecticut
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- Current CPR certification (BLS)
- At least 6 months of Nurse Practitioner experience preferred
