Experienced Nurse Practitioners are sought for per diem opportunities conducting home health risk assessments. This flexible position allows you to set your own schedule with shifts available any day of the weekMonday through Sunday. Work is performed in various locations throughout Fairfield County, including Bridgeport, Stamford, and Norwalk, with each assessment typically lasting around 45 minutes. Assignments may vary, including annual health assessments as well as post-discharge visits for Medicare Advantage plan members.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling to accommodate your availability
- Minimum commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual risk assessments in patients homes for Medicare Advantage members
- Review and evaluate prior and current medical and medication histories, as well as existing medical conditions
- Educate patients about their health conditions and medications
- Identify gaps in care and recommend solutions
- No requirement to create treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner license in Connecticut
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months of recent experience as an NP preferred
- MSN from an accredited nursing program
Compensation for this role is based on completed evaluations, ranging from $80 to $90 per visit, and includes a mileage stipend and administrative compensation.
