An excellent opportunity is available for experienced Nurse Practitioners seeking flexible, per diem work in home health assessments. This position focuses on conducting risk assessments for Medicare Advantage members throughout Oneida County, including New Hartford, Utica, Rome, Whitesboro, and New York Mills. Home visits typically last about 45 minutes, and your schedule is entirely self-directed, offering you the freedom to create your own hours between Monday and Sunday.
Schedule/Hours
- Flexible shifts available seven days a week
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities
- Perform annual and post-discharge health risk assessments for Medicare Advantage plan members in their homes
- Review and evaluate patients prior and current medical history, conditions, and medication use
- Educate patients on health conditions, wellness, and medications
- Identify care gaps and provide recommendations for follow-up
- No requirements for prescribing medications or developing treatment plans
Qualifications
- Active Nurse Practitioner license in New York
- Board Certification through AANP or ANCC
- Current CPR/BLS certification
- MSN from an accredited nursing program
- At least 6 months of experience as a Nurse Practitioner is preferred
This per diem position offers competitive compensation at $80$90 per evaluation, with additional stipends for mileage and administrative time.
