Deliver vital healthcare services as a per diem Nurse Practitioner specializing in home health risk assessments across Green County, WI. This role focuses on comprehensive evaluations of Medicare Advantage plan members located in Monroe, Brodhead, New Glarus, and surrounding areas. Each assessment typically takes about 45 minutes and can include annual health reviews or post-discharge visits. Your expertise will provide invaluable insights without the requirement to create treatment plans or prescribe medications.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to design your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review prior and current medical history, medical conditions, and medication use
- Educate members regarding their medical conditions and medications
- Identify and address gaps in care, offering recommendations for improvement
- Document findings accurately and efficiently
Qualifications:
- Current Nurse Practitioner license in Wisconsin
- Valid CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- MSN degree from an accredited nursing program
