Step into a rewarding per diem opportunity supporting Medicare Advantage plan members through comprehensive home health risk assessments. This dynamic nurse practitioner role offers the flexibility to create your own schedule and the ability to make a real impact for individuals living in Dallas County. Each assessment visit is approximately 45 minutes and may be an annual health review or a post-discharge check-in, conducted in member homes across Fordyce, Sparkman, Carthage, and other nearby areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours and booking preferences
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Complete in-home annual health risk assessments for Medicare Advantage members
- Review prior and ongoing health history, current medical conditions, and medication profiles
- Increase member knowledge by providing tailored education on conditions and prescriptions
- Identify and communicate potential gaps in care, offering actionable recommendations
- No need to develop treatment plans or prescribe medicationskeep your focus on assessment and education
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current Basic Life Support (CPR) certification
- Board certification via AANP or ANCC
- Minimum of six months nurse practitioner experience preferred
- MSN from an accredited nursing program
