Job Description

Embark on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments. You will work with Medicare Advantage plan members across Accomack County, VA, including areas such as Chincoteague, Onancock, and Parksley. This position empowers you to build your own schedule, offering flexibility while making a significant impact on patient well-being in their own homes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Autonomy to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual and post-discharge home health risk assessments
  • Review and evaluate patients prior and current medical histories, conditions, and medication records
  • Provide education to patients about their health conditions and medications
  • Identify and report any gaps in care, recommending solutions when needed
  • Documentation without development of treatment plans or prescribing responsibilities

Qualifications:

  • Active Nurse Practitioner license in VA
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • At least six months of recent experience as a Nurse Practitioner preferred
  • MSN degree from an accredited school of nursing
