Take on a valued per diem role focused on delivering high-quality care to Medicare Advantage plan members in Henry County. As a Nurse Practitioner specializing in home health risk assessments, you'll work directly with patients in settings across New Castle, Middletown, Knightstown, and surrounding areas. This position offers autonomy and flexibility, allowing you to make a meaningful impact on patient well-being while balancing your schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-schedulingchoose your own days and hours
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage plan members
• Review prior and current medical history, chronic conditions, and medication regimens
• Educate patients and/or caregivers on disease processes, health maintenance, and medications
• Identify care gaps and provide actionable recommendations to support care continuity
• No prescribing or direct treatment plan creation requiredfocus on assessment and education

Qualifications:

• Active unencumbered Nurse Practitioner license in Indiana
• Current BLS/CPR certification
• Board Certification from AANP or ANCC
• MSN degree from an accredited nursing program
• At least 6 months of recent NP experience preferred

This position provides compensation of $80$90 per evaluation, plus a mileage stipend and payment for administrative time.
