Advance patient care in a community-based setting by delivering high-quality home health risk assessments to Medicare Advantage plan members throughout Henry County, IN. This per diem opportunity offers Nurse Practitioners a flexible, independent role performing comprehensive evaluations in a supportive, dynamic environment.

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage members.
• Assess medical history, chronic and acute conditions, and medication regimens during approximately 45-minute visits.
• Educate patients about their medical conditions and medications, ensuring comprehension and adherence.
• Identify gaps in care and provide actionable recommendationswithout the need to prescribe or develop treatment plans.
• Serve residents in New Castle, Middletown, Knightstown, and surrounding Henry County areas.

Qualifications:

• Active Nurse Practitioner (NP) license in Indiana.
• Current BLS (CPR) certification.
• Board Certification from AANP or ANCC required.
• At least 6 months of recent work experience as an NP (preferred).
• MSN degree from an accredited nursing program.

In this role, youll receive compensation of $80$90 per completed evaluation, plus a mileage stipend and payment for administrative time.
