Advance your career as a Nurse Practitioner specializing in home health risk assessments across Henry County, IN. This per diem role offers maximum flexibility, allowing you to create your own work schedule while providing vital assessments for Medicare Advantage plan members in New Castle, Middletown, Knightstown, and surrounding areas. Each risk assessment visit typically lasts around 45 minutes and may include annual health assessments or post-discharge evaluations.

- Set your preferred weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive annual home health risk assessments for patients on a Medicare Advantage plan
- Review and evaluate medical, surgical, and medication histories
- Identify care gaps and provide clear recommendations for follow-up or resolution
- Deliver education to patients regarding their conditions and medications
- No responsibilities for prescribing medications or creating treatment plans
Qualifications:
- Valid Nurse Practitioner license in Indiana (IN)
- Current certification in Basic Life Support (CPR/BLS)
- AANP or ANCC board certification required
- MSN from an accredited nursing program
- At least 6 months of recent NP experience preferred
Compensation and Benefits: This opportunity offers a competitive fee of $80$90 per evaluation, plus a mileage stipend and compensation for administrative time, ensuring you are rewarded for both your clinical expertise and time spent traveling and documenting.
