A skilled Nurse Practitioner is needed for per diem work conducting home health risk assessments across Garfield County, Colorado. This is an excellent opportunity to deliver high-quality, patient-focused care directly within the home environment, with significant flexibility in your schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum required commitment on a weekly or bi-weekly basis
Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members residing in Garfield County, including Glenwood Springs, Rifle, and Carbondale
- Conduct thorough evaluations of past and current medical history, medical conditions, and medication usage
- Educate patients on their medical conditions and prescribed medications to promote understanding and improved compliance
- Identify care gaps in patient histories, and offer appropriate recommendations to address them
- Visits may include annual health assessments or post-discharge evaluations as needed
- No requirement to prescribe medications or create treatment plans
Qualifications:
- Active, unencumbered Nurse Practitioner license in Colorado
- Current CPR (BLS) certification
- Board Certification via AANP or ANCC
- Minimum of 6 months of current NP experience is preferred
- Master of Science in Nursing (MSN) from an accredited institution
This position offers compensation of $80-$90 per completed evaluation, plus a mileage stipend and payment for administrative time.
