Job Description

As a dedicated Nurse Practitioner with home health expertise, youll play a vital role in supporting Medicare Advantage members through in-home health risk assessments throughout Hartford County, CT. This highly flexible per diem position allows you to create your own schedulechoose shifts between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment to fit your lifestyle and availability.

Primary Responsibilities:

  • Conduct in-home annual health risk assessments for Medicare Advantage members
  • Review, evaluate, and document comprehensive medical, medication, and surgical history
  • Assess current conditions and identify any gaps in care
  • Educate patients on their health conditions and proper medication use
  • Offer care recommendations and resources, focusing on patient education (no treatment plan formulation or prescription expectation)
  • Ensure each assessment is thorough, compassionate, and adapts to varied visit purposes, such as post-discharge or annual evaluations

Qualifications:

  • Active Nurse Practitioner license in Connecticut
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current CPR certification (BLS)
  • Minimum of 6 months recent Nurse Practitioner experience preferred
  • Strong interpersonal, assessment, and documentation skills
