Exciting opportunity for an experienced Nurse Practitioner to support quality-centered care in home health settings throughout Callaway County. This per diem role focuses on conducting comprehensive risk assessments for Medicare Advantage plan members, typically taking about 45 minutes per visit. Each shift may include visits to residents in Fulton, New Bloomfield, Tebbetts, Mokane, and surrounding areas, with locations varying by assignment.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling to suit your availability
• Set your own minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct in-depth annual health risk assessments in patients homes
• Review and evaluate medical history, chronic and current medical conditions, and medication usage
• Deliver clear education to patients about their health status and medications
• Identify gaps in care and provide actionable recommendations to improve patient outcomes
• No prescribing or development of individual treatment plans required

Qualifications:

• Active Nurse Practitioner license in Missouri
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• Master of Science in Nursing (MSN) from an accredited institution
• Minimum six months of nurse practitioner experience preferred

This position offers compensation of $80$90 per completed evaluation, along with a mileage stipend and paid administrative time.
