Job Description

Make a meaningful impact on patient care by providing comprehensive risk assessments in a home health setting. In this per diem position, youll perform evaluations for Medicare Advantage plan members living in and around New Bern and throughout Craven County, including areas such as Havelock, Vanceboro, and Bridgeton. The Nurse Practitioner will assess patients in the comfort of their homes, helping to bridge gaps in care and improve outcomes.

Schedule:

• Flexible shifts available Monday through Sunday
• Build your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct home health risk assessments (average visit: 45 minutes) for members of a Medicare Advantage plan
• Review and document patient medical histories, prior conditions, and current medications
• Educate patients on their medical conditions, treatments, and appropriate medication usage
• Identify and address gaps in care, delivering actionable recommendations
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner License in North Carolina
• Current CPR (BLS) certification
• Board Certified via AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
