Experienced Nurse Practitioners are needed for per diem opportunities conducting home health risk assessments within Craven County. This position provides the flexibility to create your own schedule, with shifts available any day of the week. Assignments take place in the homes of Medicare Advantage plan members, with each assessment typically lasting about 45 minutes. Visits may be annual health assessments or post-discharge evaluations, focusing on locations throughout the county.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own working days and hours
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive annual risk assessments in patient homes for a Medicare Advantage health plan
• Review and evaluate medical history, diagnoses, and current medication usage
• Deliver education to members regarding their medical conditions and prescribed treatments
• Recognize gaps in care and suggest evidence-based recommendations to address them
• Document findings accurately; direct treatment planning or prescribing is not part of this role

Qualifications:

• Active Nurse Practitioner License in North Carolina
• Current CPR certification (BLS)
• Board certified via AANP or ANCC
• Minimum six months of recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited program
