Take your advanced clinical skills on the road by providing vital care services as a Nurse Practitioner specializing in home health. In this per diem opportunity, you will conduct comprehensive risk assessments for Medicare Advantage members across Craven County. With flexible, self-scheduled shifts available Monday through Sunday, you have the autonomy to tailor your work to fit your lifestyle, meeting a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

  • Complete annual and post-discharge home health risk assessments for Medicare Advantage members
  • Assess and document medical and medication history
  • Educate patients on medical conditions and proper medication use during in-home visits (average length: 45 minutes)
  • Identify gaps in care and recommend appropriate next steps for resolution
  • No development of treatment plans or prescription writing required

Desired Qualifications:

  • Active Nurse Practitioner license in the state of North Carolina
  • MSN degree from an accredited nursing program
  • Current CPR certification (BLS)
  • National Board Certification (AANP or ANCC)
  • At least 6 months of current experience as a Nurse Practitioner, preferably in home health
