Healthcare professionals with advanced clinical expertise are needed to conduct in-home health risk assessments for Medicare Advantage plan members throughout Jasper County. This per diem opportunity empowers you to design a flexible work schedule while making a positive impact on patients in their own homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own working hours
• Commitment required on a weekly or bi-weekly basis

Key Responsibilities:

• Complete comprehensive annual health risk assessments for Medicare Advantage participants
• Assess prior and current medical history, chronic conditions, and medication regimens
• Deliver patient education on medical conditions and prescribed medications
• Identify care gaps, and make recommendations to close them
• No requirement to create treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in MO
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN degree from an accredited nursing program
• At least 6 months of current NP clinical experience is preferred

This role offers compensation from $80 to $90 per evaluation completed, with a stipend to cover mileage and administrative time.

Practitioners will work in various cities within Jasper County, including Carthage, Webb City, Carterville, and Joplin. Assignments may vary by shift, offering the opportunity to serve diverse communities within the region.
