This per diem role involves conducting comprehensive home health risk assessments for Medicare Advantage members throughout Ripley County. As a clinician in this position, you will provide valuable evaluations that enhance care quality and help bridge gaps in healthcare for a diverse community.

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members in Doniphan, Naylor, Fairdealing, and surrounding areas
• Evaluate medical history, conditions, and medications to offer a complete clinical picture
• Deliver education to patients on their diagnoses and medication regimens
• Identify opportunities to improve patient care and recommend actionable steps to close care gaps
• No requirements for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Missouri
• CPR certification (BLS) is required
• Board Certification from AANP or ANCC
• Minimum of 6 months recent experience as an NP (preferred)
• MSN degree from an accredited nursing program
