Job Description
Deliver care directly to patients in their homes by conducting comprehensive health risk assessments for Medicare Advantage members throughout Ripley County, MO. In this per diem Nurse Practitioner role focused on home health, you serve the diverse communities of Doniphan, Naylor, and Fairdealing, performing assessments that inform patient-centered recommendations and support quality healthcare outcomes.
Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule based on availability
- Set a minimum weekly or bi-weekly hourly commitment according to your preference
Key Responsibilities:
- Complete annual and post-discharge risk assessments for Medicare Advantage plan members in their residences
- Review and evaluate each patients medical and medication history, documenting current and prior conditions
- Offer education and guidance about chronic health issues and medication management directly to patients
- Identify gaps in care, providing actionable recommendations to improve patient health and follow-up
- No requirement to develop treatment plans or prescribe medicationsfocus strictly on assessments and education
Qualifications:
- Active Nurse Practitioner license in the state of Missouri
- Current BLS/CPR certification
- Board Certified through AANP or ANCC
- At least six months of recent nurse practitioner experience preferred
- Masters Degree in Nursing (MSN) from an accredited institution
