As a Nurse Practitioner specializing in Home Health, you will have the opportunity to conduct comprehensive risk assessments for Medicare Advantage members, supporting patients in Franklin, Brentwood, Spring Hill, and surrounding communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Personalize your schedulework around your commitments
- Choose between a weekly or bi-weekly minimum hourly commitment
Key Responsibilities:
- Perform annual health risk assessments and post-discharge visits in patient homes
- Review and evaluate prior/current medical history, health conditions, and medications
- Offer patient education on chronic conditions and medication adherence
- Identify gaps in care and provide actionable recommendations for improvement
- Documentation and assessment only; treatment planning or prescription writing not required
Qualifications:
- Valid Nurse Practitioner license in Tennessee
- Board certification through AANP or ANCC
- Current CPR/BLS certification
- Graduation from an accredited MSN program
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation for this per diem position is offered on a per-evaluation basis ranging from $80$90. Additional perks include a mileage stipend and compensation for administrative time.
