Make a meaningful impact by providing valuable healthcare insights as an experienced Nurse Practitioner specializing in home health risk assessments. This per diem role offers you the autonomy to create your own schedule and contribute to the well-being of Medicare Advantage plan members residing throughout Williamson County, TN.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Choose minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Visit Medicare Advantage members in their homes for comprehensive risk assessments, averaging 45 minutes per visit
- Review and evaluate both prior and current medical history, ongoing medical conditions, and medication usage
- Educate members on health conditions, medications, and preventative care measures
- Identify gaps in care and offer recommendations for resolution, supporting improved patient outcomes
- No requirements for creating treatment plans or prescribing medications; your focus is strictly on assessment and education
Qualifications:
- Active Nurse Practitioner License in Tennessee
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
In this role, youll receive $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
