Experienced Nurse Practitioners are needed for per diem, home health risk assessment visits throughout Nash County, NC. In this autonomous position, you will support Medicare Advantage members with in-home health assessments. This is an excellent opportunity to tailor your work schedule and gain valuable experience in community-focused healthcare.
Schedule
- Flexible shifts available Monday through Sunday
- Design your own schedule to suit your lifestyle
- Minimum commitment on a weekly or bi-weekly basis
Key Responsibilities
- Conduct comprehensive, in-home risk assessments (approximately 45 minutes each)
- Evaluate patient history, current conditions, and medication use
- Perform visits for both annual assessments and post-discharge follow-ups
- Educate patients on disease management and medication adherence
- Identify care gaps and suggest appropriate next steps; no prescribing or treatment plan creation required
Qualifications
- Active Nurse Practitioner License in North Carolina
- Board Certification by AANP or ANCC
- Current BLS (CPR certification)
- MSN degree from an accredited nursing program
- Minimum of 6 months recent experience as a Nurse Practitioner is preferred
