Job Description

In this per diem role, you'll enjoy the advantage of flexible scheduling, setting shifts between Monday and Sunday to best fit your lifestyle with the option to create your own schedule and select your weekly or bi-weekly hourly commitments.

Responsibilities:

  • Perform home health risk assessments for Medicare Advantage plan members in their residences.
  • Deliver comprehensive annual evaluations, each averaging 45 minutes, which may include annual assessments or post-discharge visits.
  • Review and evaluate medical histories, current health conditions, and medication records.
  • Educate patients regarding their health status, chronic conditions, and prescribed medications.
  • Identify potential gaps in care and make recommendations for improved health outcomes.
  • Note: This role does not require the creation of treatment plans or the prescribing of medications.

Qualifications:

  • Active Nurse Practitioner license in NC.
  • Current CPR certification (BLS required).
  • Board Certification through AANP or ANCC.
  • MSN degree from an accredited nursing school.
  • At least 6 months of recent experience as a Nurse Practitioner (preferred).

Compensation for this position includes $80 to $90 per evaluation, along with an additional stipend for mileage and administrative time.
