Job Description
Deliver important care and support by conducting home health risk assessments for Medicare Advantage plan members across Wilson County, NC. As a Nurse Practitioner in a per diem capacity, you will perform comprehensive health evaluations in patients' homes, helping ensure their continued well-being following annual or post-discharge assessments. This position provides a high degree of autonomy, rewarding work, and a flexible scheduling experience.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete in-home health risk assessments for Medicare Advantage members
- Review patients medical history, conditions, and medication usage
- Offer education regarding ongoing medical issues and prescribed medications
- Document findings, identify care gaps, and provide actionable recommendations
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent experience practicing as a Nurse Practitioner is preferred
