Job Description

Advance your clinical expertise while enjoying a flexible, per diem schedule by performing home health risk assessments for Medicare Advantage plan members. This role empowers nurse practitioners to directly impact patient care throughout Marion County, Illinoiscovering areas such as Centralia, Salem, and Sandoval. You will work independently, conducting comprehensive assessments in patients residences and playing a crucial role in preventative healthcare.

Schedule:

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment

Primary Responsibilities:

  • Perform thorough annual home health risk assessments, visiting Medicare Advantage members in their homes
  • Obtain and evaluate prior and current medical history, conditions, and medication lists
  • Educate patients on chronic disease management and medication adherence
  • Identify care gaps and communicate actionable recommendations
  • Attend to diverse appointment types, including annual wellness checks and post-discharge visits
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Illinois
  • Master of Science in Nursing (MSN) from an accredited nursing program
  • Board Certification through AANP or ANCC
  • Current Basic Life Support (BLS/CPR) certification
  • At least 6 months of recent experience as a nurse practitioner is preferred
