Elevate your career as a Nurse Practitioner specializing in home health by performing risk assessments for Medicare Advantage plan members in Collier County. This per diem opportunity offers the flexibility to create your own schedule, with shifts available any day of the week. Youll provide critical health evaluations in Naples, Immokalee, Marco Island, and neighboring communities, supporting proactive care for a diverse senior population.
Schedule:
- Flexible shifts between Monday and Sundayset your own hours
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual home health risk assessments at members residences
- Evaluate medical history, current conditions, and medication profiles
- Educate patients on chronic conditions and ongoing medications
- Identify care gaps and make recommendations to close them
- No prescribing or treatment plan development necessary
Essential Qualifications:
- Current, unrestricted Nurse Practitioner license in Florida
- MSN from an accredited nursing program
- Board certified through AANP or ANCC
- Valid and up-to-date CPR (BLS) certification
- At least six months of current NP experience preferred
