Step into an impactful role as a Nurse Practitioner specializing in home health, providing risk assessments for Medicare Advantage plan members throughout Collier County, Florida. This per diem opportunity allows you to serve communities in locations such as Naples, Immokalee, and Marco Island, adapting your schedule to fit your lifestyle while delivering essential healthcare services.
Schedule:
- Flexible shifts available every day, Monday through Sunday
- Create your own schedule based on your availability
- Commit to a minimum number of hours weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive home health risk assessments, each averaging 45 minutes
- Evaluate patients prior and current medical histories, including all relevant medical conditions and medications
- Educate individuals regarding their health status, management of existing conditions, and medication usage
- Identify care gaps and provide actionable recommendations to promote optimal health outcomes
- No requirement for developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Current CPR certification (BLS)
- Board certification from AANP or ANCC
- At least six months of recent NP experience preferred
- MSN degree from an accredited nursing program
Youll receive compensation of $80$90 for each evaluation you complete, in addition to a mileage stipend and reimbursement for administrative time. This is a unique position for practitioners who value flexibility and autonomy while making a tangible difference in patient health.
