Job Description

Healthcare professionals with experience in home health nursing are invited to provide comprehensive risk assessments to Medicare Advantage plan members in the Dare County region. This per diem opportunity focuses on delivering high-quality care directly to patients in their residences, accommodating both annual health evaluations and post-discharge visits that typically last around 45 minutes each. Flexible scheduling is offered, allowing you to select shifts between Monday and Sunday and to determine your own weekly or bi-weekly commitment.

Schedule:

  • Flexible shift options Monday through Sunday
  • Create your own work schedule
  • Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage members in assigned locations throughout Dare County
  • Assess patients prior and current medical history, chronic medical conditions, and medication usage
  • Educate patients on their health conditions and medications
  • Identify care gaps and recommend solutions to promote wellness and improved outcomes
  • No requirement to prescribe medications or create treatment plans

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • MSN degree from an accredited school of nursing
  • Board Certification (AANP or ANCC)
  • Current CPR/BLS certification
  • Minimum of 6 months of current NP experience preferred
