Job Description

Advance your career as a Nurse Practitioner providing essential home health services on a per diem basis. This dynamic role focuses on conducting home health risk assessments for Medicare Advantage plan members throughout Oklahoma County, including Oklahoma City, Edmond, and Midwest City. Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete annual home health risk assessments during in-home visits (each typically lasting 45 minutes)
  • Evaluate patients prior and current medical history, chronic conditions, and medication usage
  • Educate members on disease management, medications, and healthy living
  • Identify gaps in care and make recommendations to address them
  • There is no requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in Oklahoma
  • Current BLS CPR certification
  • Board Certification through AANP or ANCC
  • At least 6 months of recent Nurse Practitioner experience is preferred
  • MSN degree from an accredited institution
