Advance your career as a Nurse Practitioner providing essential home health services on a per diem basis. This dynamic role focuses on conducting home health risk assessments for Medicare Advantage plan members throughout Oklahoma County, including Oklahoma City, Edmond, and Midwest City. Schedule:
- Flexible shifts available Monday through Sunday
- Opportunity to create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments during in-home visits (each typically lasting 45 minutes)
- Evaluate patients prior and current medical history, chronic conditions, and medication usage
- Educate members on disease management, medications, and healthy living
- Identify gaps in care and make recommendations to address them
- There is no requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Oklahoma
- Current BLS CPR certification
- Board Certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience is preferred
- MSN degree from an accredited institution
