Step into an impactful per diem opportunity focused on improving patient lives through comprehensive home health risk assessments. As a Nurse Practitioner, you will travel within Muskogee County, including areas such as Muskogee, Fort Gibson, and Warner, providing essential healthcare support to Medicare Advantage plan members.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling to fit your lifestyle
- Minimal weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct home health risk assessments, lasting approximately 45 minutes, on Medicare Advantage members in various communities within Muskogee County
- Perform thorough evaluations of prior and current medical history, existing conditions, and medication use
- Educate patients on their medical conditions and prescribed medications
- Pinpoint gaps in care and share actionable recommendations to enhance patient outcomes
- Note: This position does not involve developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Oklahoma
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Preferred: 6+ months of current Nurse Practitioner experience
- Master of Science in Nursing (MSN) from an accredited institution
