As a Nurse Practitioner with expertise in home health, you will play a vital role in supporting patient well-being and closing key gaps in care. This per diem opportunity offers autonomy and flexibility, making it an ideal fit for professionals seeking a personalized schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Manage your own hours and build a schedule that works for you
- Minimal weekly or bi-weekly hour commitments
Key Responsibilities:
- Perform comprehensive annual risk assessments in patient homes
- Review and evaluate prior and current medical conditions, history, and medication regimens
- Provide education to patients regarding their medical conditions and prescribed medications
- Identify and address any gaps in care, offering actionable recommendations to enhance patient outcomes
- Visits typically last approximately 45 minutes
- No requirement for treatment plan development or prescription management
Required Qualifications:
- Active Nurse Practitioner (NP) license in Oklahoma
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing school
- Minimum 6 months recent experience as a Nurse Practitioner preferred
