Job Description

As a Nurse Practitioner with expertise in home health, you will play a vital role in supporting patient well-being and closing key gaps in care. Schedule:

  • Flexible shifts available Monday through Sunday
  • Manage your own hours and build a schedule that works for you
  • Minimal weekly or bi-weekly hour commitments

Key Responsibilities:

  • Perform comprehensive annual risk assessments in patient homes
  • Review and evaluate prior and current medical conditions, history, and medication regimens
  • Provide education to patients regarding their medical conditions and prescribed medications
  • Identify and address any gaps in care, offering actionable recommendations to enhance patient outcomes
  • Visits typically last approximately 45 minutes
  • No requirement for treatment plan development or prescription management

Required Qualifications:

  • Active Nurse Practitioner (NP) license in Oklahoma
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN degree from an accredited nursing school
  • Minimum 6 months recent experience as a Nurse Practitioner preferred
