Schedule:
- Flexible shifts available any day between Monday and Sunday
- Set your own work hours
- Minimum weekly or bi-weekly commitment required
Key Responsibilities:
- Perform comprehensive annual risk assessments in patients homes within Muskegon County
- Review and evaluate medical histories, current conditions, and medication usage
- Educate patients about their health conditions and medication regimens
- Identify gaps in care and offer recommendations to close those gaps
- Focus on assessment onlyno requirement to write treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in the state of Michigan
- Current CPR (BLS) certification
- Board certified by AANP or ANCC
- Minimum of 6 months current Nurse Practitioner experience preferred
- MSN from an accredited nursing program
