Job Description

Great opportunity for an experienced Nurse Practitioner seeking a flexible, rewarding position in home health. This per diem role involves conducting comprehensive risk assessments for Medicare Advantage plan members throughout Muskegon County. Each visit is approximately 45 minutes, and assignments may include annual wellness checks or post-discharge assessments. Practitioners will travel to members residences in locations such as Muskegon, Norton Shores, and Muskegon Heights, supporting at-risk populations and contributing to vital continuity of care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own scheduletailor your work around personal commitments
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Complete in-home health risk assessments for Medicare Advantage members
  • Review and evaluate prior/current medical histories, conditions, and medications
  • Offer patient education on health conditions and medication adherence
  • Identify gaps in care and suggest resolutions to help close those gaps
  • Visits are evaluation-focused; no treatment planning or prescribing required

Qualifications:

  • Current Nurse Practitioner license in Michigan
  • Active CPR/BLS certification
  • MSN degree from an accredited nursing school
  • Board certification through AANP or ANCC
  • At least 6 months of recent experience as an NP preferred

Compensation & Perks:

  • Receive $80$90 for each completed evaluation
  • Mileage stipend and administrative time included
