Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Set your own work hours
  • Minimum weekly or bi-weekly commitment required

Key Responsibilities:

  • Perform comprehensive annual risk assessments in patients homes within Muskegon County
  • Review and evaluate medical histories, current conditions, and medication usage
  • Educate patients about their health conditions and medication regimens
  • Identify gaps in care and offer recommendations to close those gaps
  • Focus on assessment onlyno requirement to write treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in the state of Michigan
  • Current CPR (BLS) certification
  • Board certified by AANP or ANCC
  • Minimum of 6 months current Nurse Practitioner experience preferred
  • MSN from an accredited nursing program
