This opportunity suits skilled Nurse Practitioners with a passion for home health and preventive care, focusing on risk assessments for Medicare Advantage members in Hertford County, NC, including areas such as Ahoskie, Murfreesboro, and Winton.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to create your own work schedule
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Review and document patients medical histories, existing conditions, and medication lists
- Offer education and clarify medication routines and medical conditions to members in-home
- Identify potential care gaps, making actionable recommendations for continuity and comprehensive care
- Enjoy a focus on assessmentno requirement for treatment planning or prescribing medications
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board certification through AANP or ANCC
- Current CPR (BLS) certification
- MSN from an accredited nursing program
- At least 6 months of Nurse Practitioner experience preferred
