Looking for a rewarding per diem opportunity in home health care? Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage members throughout Delaware County, including Muncie, Yorktown, and Chesterfield. This role allows you to manage your own schedule, delivering high-quality care in a flexible and autonomous environment.

Schedule:

• Flexible shifts offered Monday through Sunday
• Create your own work schedule
• Commit to a minimum of weekly or bi-weekly hours according to your availability

Responsibilities:

• Conduct comprehensive annual risk assessments in patient homes for a Medicare Advantage health plan
• Review and evaluate prior and current medical history, medical conditions, and medication regimens
• Educate patients on their medical conditions and prescribed medications
• Identify gaps in care and provide expert recommendations to address them
• Note: No development of treatment plans or medication prescribing is required

Qualifications:

• Active Nurse Practitioner license in Indiana (IN)
• Board Certification from AANP or ANCC
• Current BLS/CPR certification
• MSN degree from an accredited nursing program
• At least 6 months recent experience as a Nurse Practitioner preferred
