Deliver high-quality patient care in a flexible, per diem role focused on home health risk assessments. Utilize your clinical expertise as a Nurse Practitioner by visiting Medicare Advantage plan members in Delaware County. Each visit, lasting about 45 minutes, provides an opportunity to significantly impact patient outcomes through thorough healthcare evaluations. Youll engage with individuals in a variety of settings across areas such as Muncie, Yorktown, and Chesterfield. Experience autonomy as you set your own schedulechoose when and how often you work, with flexible shifts available throughout the week.

Schedule:

• Flexible shift options available Monday through Sunday
• Design your own schedule
• Commit to a minimum number of hours weekly or bi-weekly

Primary Duties:

• Perform comprehensive home health risk assessments for Medicare Advantage participants
• Review medical histories, current health status, and medication profiles
• Educate patients on health conditions, medication management, and preventive measures
• Detect care gaps and make recommendations to enhance patient care
• Note: No prescribing or treatment planning responsibilities required

Preferred Qualifications:

• Valid Nurse Practitioner license in Indiana
• Board Certification (AANP or ANCC)
• Current CPR/BLS certification
• MSN degree from an accredited program
• At least 6 months of Nurse Practitioner experience is preferred

Compensation for this per diem role includes $80$90 per completed evaluation, plus reimbursement for mileage and compensation for administrative time.
