Working throughout Delaware County, you'll visit Medicare Advantage plan members for comprehensive health risk assessments in their own homes. This flexible position lets you design your own schedule, offering genuine work-life balance.
Schedule:
- Choose your own shifts between Monday and Sunday
- Pick your available hours for a weekly or bi-weekly commitment
Key Responsibilities:
- Perform annual home health risk assessments with visits averaging 45 minutes each
- Travel throughout Center Township and surrounding areas such as Muncie, Yorktown, and Chesterfield as needed
- Assess medical history, current health conditions, and medication usage
- Educate patients about their diagnoses and treatments
- Identify gaps in care and make recommendations to enhance overall health outcomes
- No prescribing, treatment planning, or follow-up care management required
Qualifications:
- Valid Nurse Practitioner License in the state of Indiana
- Current BLS (CPR) certification
- Board Certification through AANP or ANCC
- Six months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited institution
Compensation for this position ranges from $80 to $90 for each evaluation completed, with an additional mileage stipend and compensation for administrative time.
