Job Description

Take on a valuable role as a Nurse Practitioner specializing in home health risk assessments. In this per diem opportunity, youll serve Medicare Advantage plan members across areas within Delaware County, including Muncie, Yorktown, and Chesterfield. Your visits, averaging 45 minutes, may involve annual health assessments or post-discharge reviews, tailored to the diverse needs of each patient.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own schedule to fit your lifestyle
  • Choose either a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage beneficiaries
  • Review prior and current medical history, existing conditions, and medication usage
  • Educate members on their health conditions and prescriptions, supporting their understanding and wellbeing
  • Identify gaps in care and deliver actionable recommendations to enhance patient outcomes
  • No treatment planning or medication prescribing is necessary for this position

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current BLS certification (CPR)
  • Board certified by AANP or ANCC
  • At least 6 months recent experience as an NP preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Compensation for this position is highly competitive, with $80-$90 per evaluation provided, in addition to compensated mileage and administrative time.
