Job Description

Seeking a skilled Nurse Practitioner to deliver high-quality home health risk assessments for Medicare Advantage plan members throughout Elmore County. In this per diem position, you'll play a key role in improving patient outcomes through thorough medical evaluations in residents homes.

Schedule:

  • Flexible shifts available seven days a week
  • Self-schedulingcontrol your own calendar
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities include:

  • Conducting comprehensive annual home health risk assessments for Medicare Advantage members
  • Reviewing prior and current medical history, chronic conditions, and medication usage
  • Educating members about their medical conditions and prescribed medications
  • Identifying care gaps and recommending appropriate follow-up actions
  • Visits typically last approximately 45 minutes per patient; no treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Board certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN from an accredited nursing program
  • At least 6 months of recent NP experience preferred

As a Home Health Nurse Practitioner, you'll travel within areas such as Mountain Home and Glenns Ferry, providing crucial assessments to members in need. In addition to your evaluation fee, youll receive a mileage stipend and administrative time compensation, supporting both your expertise and travel.
