Job Description
Embark on a rewarding per diem opportunity as a healthcare professional delivering essential home health risk assessments for Medicare Advantage plan members in Elmore County, ID. This role offers a high degree of autonomy, flexible scheduling, and the chance to make a meaningful impact by visiting individuals in their homes throughout Mountain Home, Glenns Ferry, and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to accommodate personal and professional commitments
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete thorough home health risk assessmentseach typically lasting 45 minutes
- Review and document patients medical histories, existing conditions, and medication usage
- Educate patients on their health conditions and medications to empower better self-care
- Identify and report any gaps in preventive or ongoing care and suggest actionable interventions
- Focus exclusively on assessment and education, with no prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner (NP) licensure in Idaho
- Current BLS (CPR) certification
- Board certified through AANP or ANCC
- At least 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
Compensation for this role includes $80-$90 per evaluation performed, with additional stipends for mileage and administrative time.
