Job Description

Embark on a rewarding per diem opportunity as a healthcare professional delivering essential home health risk assessments for Medicare Advantage plan members in Elmore County, ID. This role offers a high degree of autonomy, flexible scheduling, and the chance to make a meaningful impact by visiting individuals in their homes throughout Mountain Home, Glenns Ferry, and surrounding areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to accommodate personal and professional commitments
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete thorough home health risk assessmentseach typically lasting 45 minutes
  • Review and document patients medical histories, existing conditions, and medication usage
  • Educate patients on their health conditions and medications to empower better self-care
  • Identify and report any gaps in preventive or ongoing care and suggest actionable interventions
  • Focus exclusively on assessment and education, with no prescribing or treatment planning required

Qualifications:

  • Active Nurse Practitioner (NP) licensure in Idaho
  • Current BLS (CPR) certification
  • Board certified through AANP or ANCC
  • At least 6 months of recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program

Compensation for this role includes $80-$90 per evaluation performed, with additional stipends for mileage and administrative time.
