A rewarding opportunity is available for an experienced Nurse Practitioner eager to deliver home health risk assessments for Medicare Advantage plan members throughout Barry County. In this per diem role, you will provide essential health evaluations in patient homes, supporting better outcomes for individuals and the broader community. Your knowledge and compassion will directly contribute to improved continuity of care, while offering flexibility and independence in your schedule.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Set your own working hours
- Minimum commitment required either weekly or bi-weekly
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review patients prior and current medical histories, ongoing medical conditions, and medication usage
- Offer education to patients regarding their health conditions and prescribed medications
- Identify gaps in care and recommend appropriate follow-up actions
- No requirement to create treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in Missouri
- Current CPR certification (BLS)
- Board certified through AANP or ANCC
- Minimum of six months recent NP experience preferred
- Graduate of an accredited MSN program
This unique role compensates $80$90 per completed evaluation and includes a mileage stipend along with paid administrative time. Locations span Barry County, with visits in areas such as Monett, Savannah, Country Club, Fillmore, or surrounding communities.
